The following information must be provided for registration at Lakeside High School:
1. Registration forms that must be completed are linked at the bottom of this page.
2. Withdrawal Form from previous school
3. Proof of Residency (power bill or water bill in parent/guardian's name)
4. Copy of Birth Certificate
5. Copy of Social Security Card
6. Recent report card or copy of transcript
7. Eye, Ear, and Dental screening (must be on Georgia Form #3300)
8. Certificate of Immunization (must be on Georgia Form #3032)
(The Eye, Ear, and Dental screening and the Georgia Form for Immunizations may be obtained from your personal physician or the Martinez/Evans Clinic, 868-3330, and are due within 30 days of enrollment.)
Please call Mrs. Sharon Lee at 868-3669 ext 209 or email her at sharon.lee@ccboe.net to make an appointment with a counselor to properly enroll your child. Drop-in visits are discouraged so that counselors have time to appropriately discuss your child's educational plans.
